There are clear associations between social determinants—such as a lack of safe and affordable housing, food insecurity, quality education, transportation, economic opportunity, and isolation—and health status among children, adults, and seniors. This summit aims to more fully engage the health care industry in the discussion of social determinants of health and in developing and implementing data-driven, community-centered solutions to prevent long-term chronic health conditions in the populations we serve. The summit aims to bring together health care leaders, faith-based organizations, researchers, clinicians, and other representatives from the nonprofit and public sectors to share best practices, offer community connections and resources, and discuss emerging approaches to addressing the social determinants of health.
